The hub axles are measured over the threads and the 3 most common sizes are 9mm, 9.5mm(3/8") and 10mm and come in 1mm and either 24 or 26TPI threading.
The washer does spin independently of the axle nut.

Metal is a bit soft, and it knicks easy if you are using a narrow 15mm cone wrench. But at $1.80, who cares! It works great.
