The Shimano CB90 cable adjuster and quick release is for use with Dura-Ace BR-9010R brakes, or other direct mount calipers that lack barrel adjusters or cable quick release levers. These allow you to make cable tension adjustments, and open the calipers for easy wheel removal and installation.

A number of brake line ups, notably a lot of Shimano using cantilevers, do not have a quick release in their brake systems, making tire changes a real PAIN. This is an excellent solution that I applied to a Soma Double-Cross with this problem. While pricey, this unit is built like a tank and will give years of service. It's pretty quick to install, and less costly than any other solution I could find.
